On October 1st, we transitioned to a new patient portal.
This upgrade is part of our ongoing effort to improve our services and the care we provide to you. This patient portal has useful features, like online billing, easy messaging with your provider, appointment information, and quick access to your medical records.

If you'd like to access health information and records in your Elation Passport portal, you can login below. It's important to note that you can no longer message your provider in this portal.
