Your Initial Visit

Welcome! We’re so excited to be part of your health journey. At Herself Health, we specialize in care designed for women on Medicare. Our goal is to keep you feeling your best with personalized, compassionate care that puts you first.

Why You’ll Love It Here

We’re not your typical Primary Care practice, here’s how we’re different:

Your voice matters.

We’ll listen to your concerns and work with you to create a care plan that feels just right for you.

Unrushed, compassionate care.

We take the time to listen and include you in decisions about your health, so you feel confident in your care every step of the way.

Preventive care, tailored to you.

We focus on proactive, preventive care when you need it, to help you stay healthy and address potential issues early.

Here’s What to Expect at Your First Visit

Your first visit is focused on building a strong foundation for your care and will include:

1

When You Arrive

You’ll be welcomed by an Insurance Support Specialist who will help you get settled, answer insurance questions, and share more about our practice.

2

Getting Ready for Your Provider

A Medical Assistant will bring you to your room, check your vitals, review your medications and allergies, and ask about any specific health concerns you’d like to discuss.

3

Time With Your Provider

During your visit, you’ll spend about 60 minutes with your provider talking through your health history, current concerns, medications, and broader life goals. This is also time for you and your provider to get to know each other. Together, you’ll create a personalized care plan based on your needs.

4

Before You Leave

A Medical Assistant will help with any next steps, such as lab orders or blood draws, and will schedule your follow-up visit or Annual Wellness Visit if needed.

Please Note: Your provider will perform a brief physical to assess your health and establish care, but this visit will not include a full physical or Annual Wellness Visit, so you won’t need to change into a gown. There’s no need to fast ahead of time. If fasting labs are needed, we’ll schedule those separately.

Preparing for Your Visit

To help us provide the best care, please be aware of the following:

What to Bring

Please bring your insurance card, a photo ID, and a list of any medications, vitamins, and supplements you’re taking, including dosages.

New Patient Forms

We’ll send you a link to your new patient forms 7 days before your visit. Filling them out early helps your provider spend more time focusing on you!

Billing Information

Your visit will be billed as a standard office visit, and any copay listed on your insurance card will be due at the time of your appointment.

What Our Patients Are Saying

Quality of life is important to me. I'll be 70 in less than three months. I know certain things will happen to my body.

Carolyne

Patient

Specialized for women draws my attention. It would be a good thing because over 50, there are different issues from when we were 30 and 40.

Bonnie

Patient

Doctors should follow up and listen. I shouldn't have to repeat my issues every time I see you.
